Application Form

All applicants will be required to complete a program-focused online Angel tutorial. Several resources are available to prospective students to familiarize themselves with the Angel system.  A demo course has been set-up to show what an Angel course is like and the features that are available. Another site maintains several tutorials allowing prospective students to learn the Angel system. Upon receipt of all application documents, applicants will be emailed a log-in ID and password to access the program's tutorial. If you have not received this access - it means that we have not received all your materials. All components of the tutorial must be completed before your application will be considered. Materials should be forwarded 519 West Fee Hall, East Lansing, MI 48824. Applicants may be required to participate in an interview process. 

The application period for Fall semester 2008 is closed. We are no longer accepting applications. If you wish to apply for Fall semester 2009 - check back in November for application details and deadlines.

Application Materials:

Application for the MPH graduate degree program requires the following items:

      *Medical College Admissions Test (MCAT) may substitute for GRE  

 

Students pursuing the Graduate Specialization must provide:

  • Evidence of good academic standing from their primary unit 
  • Copy of graduate school application form
  • Letter of recommendation from their advisor
  • Personal essay describing interest/experience in public health discipline and career goals
  • Resume or c.v.
  • PPH  Supplemental Information Form

 

Students interested in the Certificate option through Lifelong Learning must provide the following items. Please be aware that for processing reasons, we will ask you to print a copy of your completed lifelong education application and send an extra hard-copy to us along with your other documents at this time.
